TO PREPARE:
Select a program at a human services agency that could benefit from a program evaluation. You may wish to use the program that you have in mind for your Final Project.
Review the logic model template on p. B1 of the resource by Shakman and Rodriguez. Use this framework as a starting point to develop a logic model for an evaluation of your agency/program.
Consider the resources available, program strategies and activities, outputs, short- and long-term outcomes, and long-term impacts anticipated for your selected program. If there are other factors or categories that should be included in a program evaluation for your program/agency, adjust your logic model framework as needed. (It does not need to mirror the template on B1.)
THE ASSIGNMENT:
Develop a logic model for your selected agency/program.
Include a problem statement and assumptions.
State the inputs, activities, outputs, outcomes, and any other factors anticipated for your selected program as appropriate.
Organize your logic model creatively using graphics, flowcharts, or other visuals to support your textual information about the program.
