Each organization has a discharge process, which you will discuss in your presentation. Some problems do exist with every discharge process. Therefore, you will discuss the existing problem (s) and formulate a new process with rationales. The process chart should reflect a timeline of how this new process will take to be fully effective; also, keep in mind any time frames you may need to include to re-evaluate this new process to ensure it is being implemented effectively by your organization’s staff.

Develop a PowerPoint presentation addressing the following components.
1. Identify the current discharge process for a healthcare organization. Name: Veterans Health Administration hospital discharge process will be ok most healthcare have similar discharge processes.
2. Identify problems in this process.
3. Develop a new process with rationales for the changes.
4. Include a process chart to depict this new process.

Please include the following as some problems with discharge processes.
• Discharge discussions are not done by our providers, making it difficult to ask their health questions or voice their concerns freely.
• Patients do not understand follow-up care plans and discharge instructions even though discharge plans are supposed to start from the first day of admission. (Please include references to back these up)
• Also we do not have a discharge planner team, so floor nurses are responsible for it, increasing their workload. This leads to a limited time spent for discharge discussion which might not yield the best results for the discharged patients.
• Also effective communication is crucial for patients to understand the discharge instructions. In addition, follow-up appointments presented during the discharge discussion can be overwhelming for the patient especially if they don’t know why and how to handle it. This makes the process seem ineffective. (Please use articles to support these points above and add more processes).
